Ogden Clinic and Cache Valley Doctor Become Utah’s First Charter Member of the Cardiometabolic Center Alliance

The Cardiometabolic Center Alliance (CMCA), founded by Saint Luke’s Mid America Heart Institute, is honored to announce the addition of Ogden Clinic as a Charter member, and the first member in Utah. The Cardiometobolic Alliance Center is a national collaborative of healthcare organizations committed to the mission of improving the quality of care and outcomes in patients with cardiometabolic disease. Ogden Clinic joins a highly exclusive group of just 20 centers, bringing an advanced, patient-centered model to Utah for the first time.

“The Cardiometabolic Alliance is a group of clinics—only 20 in total—that focuses on diabetes and cardiometabolic health,” said Dr. Clark. “It started at Saint Luke’s, where patients could get all their care needs met in one place.”

The CMCA model of care is centered on three key overlapping conditions:

Diabetes and Cardiovascular Disease

Diabetes and Kidney Disease

Diabetes and Heart Failure

These conditions are among the most prevalent and life-threatening chronic diseases in the U.S. The Alliance’s model focuses on guideline-directed therapy, comprehensive risk reduction, and a team-based, patient-first approach. It has been shown to improve patient outcomes significantly. ​

Ogden Clinic in Logan, Utah, offers integrated services for diabetes, heart failure, obesity, kidney disease, blood pressure management, insulin care, and more, all under one roof. Patients will also have access to pharmacists, care guides, and direct connections to specialists. “Here in the clinic, patients will get their diabetes care, kidney care, blood pressure care, obesity care, insulin care, and heart failure care all in one place,” Dr. Clark said. “We’re focused on removing barriers and making it easier for patients to get the best care possible.”

Although this is the first CMCA Charter in Utah, Dr. Clark and Ogden Clinic hope it won’t be the last.“This is just the beginning,” Dr. Clark added. “We have several providers at Ogden Clinic who are interested in implementing this model. Our goal is to expand access so that every patient, regardless of insurance status or circumstances, can receive the care they deserve.”

Dr. Clark also emphasized that patients do not need to switch their primary care provider to benefit from this program. “We’re here to do the heavy lifting to get patients on guideline-directed therapy,” he said. “Once that’s established, we’ll work with their existing care team and continue to be a resource for long-term success.”
